The Primary Care Coalition of Montgomery County (PCC) is making health care happen. PCC works with clinics, hospitals, health care providers, and other community partners to coordinate health services for our most vulnerable neighbors. Our vision is a community in which all residents will have the opportunity to live healthy lives. The PCC is a 501(c)(3) nonprofit organization.
The PCC works with community partners throughout the National Capital Region to coordinate a community based healthcare system, striving for health equity and universal access to care.
Addressing breast health care disparities in the National Capital Area fits perfectly with PCC’s mission. Breast Cancer is a leading killer of women in the US with higher incidence and mortality rates for low-income populations. The PCC improves health outcomes and reduces disparities by developing data-driven interventions, engaging in continuous process improvement, and strengthening partnerships to foster and share improvements.
